Greater Manchester Mental Health NHS Foundation Trust (GMMH), formerly known as Greater Manchester West Mental Health NHS Foundation Trust (GMW), is a statutory body which, as of 1 January 2017, brings together the expertise of GMW and Manchester Mental Health and Social Care Trust.

The Trust provides inpatient and community-based mental health care for people living in Bolton, the city of Manchester, Salford and Trafford and a wide range of specialist mental health and substance misuse services across Greater Manchester, the north west of England and beyond.

Job Description
Job title: GM Police Custody Health Liaison and Diversion Facilitator Role Summary: Provide person‑centred health, social care, and diversion support to individuals in Greater Manchester Police custody suites. Work within a multidisciplinary team to assess needs, develop personalized care plans, and refer to appropriate services, aiming to reduce re‑offending and improve health outcomes. Expectations: - Operate on a 7 am–7 pm rota covering all week, reporting promptly to custody officers and team leads. - Maintain confidentiality and abide by enhanced DBS and NPPV requirements. - Demonstrate empathy, respect for lived experience, and a non‑judgemental stance. - Uphold organisational quality and safety standards. Key Responsibilities: - Conduct assessments of custody residents (min 10 yrs) to identify health, mental health, diversity, disability, and social needs. - Obtain and document consent, creating individualized “my plan” that reflects strengths and choices. - Foster hope and confidence in personal well‑being and security. - Signpost, refer, and liaise with health, social care, community, and voluntary agencies. - Provide staff briefings and information to custody colleagues and the wider system. - Maintain accurate records in police and NHS systems; complete required case notes and audit reports. - Participate in structured supervision, training, and team meetings. Required Skills: - Strong communication and interpersonal skills, capable of building trust with diverse populations. - Cultural competence and awareness of mental health, addiction, and social determinants of health. - Ability to work independently under pressure while maintaining accuracy. - Problem‑solving and case‑management aptitude. - Familiarity with confidentiality regulations, enhanced DBS, and NPPV processes. Required Education & Certifications: - Minimum of 2 years experience in health or social care (e.g., mental health, community nursing, social work, or allied health). - Relevant qualification such as NVQ Level 3 in Health and Social Care or equivalent. - Current enhanced DBS clearance and successful NPPV (minimum 3 years UK residency). - Knowledge of police custody procedures and related UK statutory frameworks is desirable.
