Job Description
Job Title: Supervisor, Home Support Role Summary: Oversee and support a team of paraprofessional home care staff to deliver high‑quality, client‑centered care. Ensure compliance with regulatory standards, promote continuous professional development, and drive quality improvement and safety initiatives. Expectations: - Lead by example in clinical and service delivery standards. - Maintain accurate documentation and regulatory compliance. - Foster a collaborative, positive work environment that encourages professional growth. Key Responsibilities: - Conduct holistic client assessments and schedule ongoing reassessments to optimize outcomes. - Develop and review inter‑professional, goal‑oriented care plans in partnership with clients and families. - Monitor and adjust client/paraprofessional assignments to maximize safety and satisfaction. - Supervise paraprofessional staff, ensuring adherence to federal, provincial, and company policies. - Escalate clinical or risk issues beyond supervisory scope. - Deliver timely feedback and coaching to enhance staff performance. - Plan and facilitate in‑service education, including evidence‑based training and continuing education participation. - Liaise with clinical quality, health informatics, and privacy teams for support and issue resolution. - Manage client complaints, ensuring timely documentation and resolution. - Coordinate recruitment, orientation, training, performance reviews, and modified work programs. - Participate in business development activities and contractual agreements for service delivery. - Represent the organization in community health and business committees. - Lead or support continuous quality improvement projects aligned with the Quality Management System. - Ensure confidentiality and cybersecurity of client and corporate data. - Comply with health and safety policies, particularly infection prevention and control. Required Skills: - Strong leadership and team management. - Knowledge of home care delivery models and regulatory requirements. - Excellent communication and interpersonal skills. - Ability to coach, mentor, and develop staff. - Proficiency in health information systems and quality improvement methodologies. Required Education & Certifications: - Bachelor’s degree in Nursing, Health Services Administration, or related field (or equivalent experience). - Current provincial nursing or paraprofessional certification required. - Certified in CPR/First Aid and, optionally, in Home Care or Community Health.
