Job Description
Job Title: Band 6 Custody Practitioner – Health and Justice Vulnerability Service Role Summary: Provide comprehensive health and social care for individuals in custody, supporting vulnerable cohorts to reduce health inequalities, facilitate diversion from the criminal justice system, and promote positive outcomes through collaboration with partner agencies. Expectations: 12 + months post‑qualification experience in varied settings; strong team player with autonomous working ability; proven assessment and risk formulation skills; ability to identify, refer, and manage complex care needs (mental health, substance misuse, homelessness, learning disability, youth vulnerability). Key Responsibilities: - Deliver frontline health and support services across custody suites and ad‑hoc sites. - Conduct thorough assessments of vulnerable individuals to inform clinical judgments and risk plans. - Identify and coordinate onward referrals to health, social care, and third‑sector services. - Collaborate with police, social services, and other partners to facilitate diversion and reintegration. - Provide case management, crisis support, and continuity of care within the care pathway. - Monitor and document clinical outcomes, contributing to service evaluation and improvement. Required Skills: - Clinical assessment, risk formulation, and decision‑making expertise. - Excellent communication and interpersonal skills for multidisciplinary collaboration. - Flexible, proactive, and self‑directed working style. - Cultural competence and understanding of diverse, deprived populations. Required Education & Certifications: - Registered with a relevant professional body (e.g., Nursing, Social Work, OT, Allied Health). - Minimum of Band 6 professional qualification and statutory registration. - Demonstrated post‑qualification experience (≥12 months) in healthcare or social care environments.
