Job Title: Senior Occupational Therapist – Frailty Role Summary: Deliver comprehensive occupational therapy assessment and intervention for frailty patients admitted through the Frailty Assessment Unit and SDEC, support MDT discharge planning, collaborate with community partners, deputise Team Leader, and mentor junior staff. Expectations: • Manage a daily clinical caseload, diagnosing and formulating individualized treatment plans. • Provide specialized therapy advice to the multidisciplinary team (MDT). • Coordinate with community partners to ensure smooth transfer of care. • Deputise for the Team Leader on clinical and managerial matters. • Lead training and supervision of new graduates, students, apprentices, and support workers. Key Responsibilities: 1. Assess patients in the Frailty Assessment Unit and Frailty SDEC area. 2. Develop and implement therapeutic interventions tailored to frailty needs. 3.
