Job Description
**Job title:** Advanced Pharmacist – Acute Care **Role Summary:** Provide specialized clinical pharmacy services across acute wards, emergency, short‑stay and ambulatory units, ensuring safe, effective, cost‑efficient medication use in line with national standards. **Expectations:** Deliver daily ward rounds, comprehensive medication reviews, and patient counselling. Lead clinical governance, multidisciplinary collaboration, and training of pharmacy staff and trainees. Contribute to audit, research, and quality improvement initiatives. **Key Responsibilities:** - Conduct individualized medication reviews during ward rounds and advise on therapy optimisation. - Ensure prescriptions comply with clinical and safety guidelines. - Coordinate interdisciplinary meetings for discharge planning and medication reconciliation. - Provide technical support and training to junior pharmacists and pharmacy technicians. - Supervise trainee pharmacists and pharmacy students. - Lead clinical pharmacy audits, research projects and quality improvement activities. - Support implementation of electronic patient record systems for medication documentation. **Required Skills:** - Clinical pharmacy expertise in acute care settings. - Strong analytical and patient assessment abilities. - Excellent communication and teamwork skills. - Leadership and mentoring competencies. - Proficiency with electronic health records, pharmacy information systems, and audit tools. **Required Education & Certifications:** - Qualified pharmacist (UK: PGCert/PGDip/PGD); required registration with the General Pharmaceutical Council. - Experience in acute care pharmacy practice, preferably within a hospital setting. - Evidence of participation in clinical governance or quality improvement projects.
