START is a Salford based mental health charity that uses creativity, connection and recovery interventions to help improve people's lives. We are passionate about providing the very best support for the people in our community who we are lucky enough to meet through our work. All of our work is directed towards improving the health and wellbeing of target groups within the local community, in particular people who are at risk of experiencing mental health difficulties. We offer a number of specific programmes, all of which help people to recognise their potential, reconnect with others and integrate back into their neighbourhoods; providing onward pathways into a network of community assets to support them to engage in further training, education, employment, volunteering and leisure pursuits. Job Description
**Job Title:** Community Connector – Wellbeing Matters **Role Summary:** Act as a frontline link between health professionals, patients and community resources within the Wellbeing Matters Social Prescribing Programme. Provide individualized, strengths‑based support to residents in Walkden & Little Hulton and, separately, to adults in a specialist mental health inpatient setting at Salford Royal Hospital, facilitating access to local services, activities and peer groups to enhance health, wellbeing and independence. **Expectations:** - Maintain professional, caring demeanor with all service users. - Demonstrate organisational capability, resilience and the ability to work within a multidisciplinary team. - Adhere to safeguarding and statutory reporting protocols. - Complete timely documentation and reporting for NHS, local authority and programme tracking. **Key Responsibilities:** 1. Conduct one‑to‑one assessments to identify individual needs, goals and circumstances. 2. Co‑create personalised action plans using strengths‑based approaches. 3. Connect users to appropriate local voluntary, community or social enterprise services and activities. 4. Manage onward referrals, ensuring smooth hand‑offs and service uptake. 5. Monitor progress, review outcomes and adjust plans as required. 6. Identify gaps in local service provision and contribute findings to delivery‑team meetings. 7. Safeguard users, report concerns per policy and statutory requirement. 8. Maintain accurate records in electronic health systems and programme databases. 9. Liaise with local providers to strengthen referral pathways and service quality. **Required Skills:** - Strong interpersonal and communication skills. - Ability to work autonomously and in a team environment. - Knowledge of community resources, local support services and social prescribing models. - Proficiency with electronic record‑keeping and basic IT skills. - Cultural competence and sensitivity to diverse populations. - Problem‑solving and resource‑identification abilities. **Required Education & Certifications:** - Minimum Level 3 qualification or equivalent experience in health, social care or related field. - Awareness of NHS safeguarding policies and local mental health service structures. - Completion of statutory safeguarding and health‑and‑safety training required for the role.
