Job Overview

This post has been developed to offer the opportunity for a qualified nurse to develop their clinical skills within a Multi-Disciplinary Community Learning Disability Team. The nurse will work under the supervision of a Band 6 Nurse and contribute to the effective operations of the specialist healthcare community service. The individual will be able to develop their professional and clinical role whilst recognising the scope of their practice and the need for supervision.

The individual will support and assist the nursing team to carry out routine nursing interventions within a variety of community and health service facilities. The nurse will work without direct supervision and within the confines of the Care Plan, when urgency dictates the application of prerogative and personal autonomy/accountability.

This post is fixed term/secondment for 4 months to 31/03/2026 to cover maternity leave. If you are interested in applying for this as a secondment position, you must obtain permission from your current line manager prior to applying for this post

Main duties of the job

Have continuing responsibility for a caseload, including the assessment of care needs and the development, implementation and evaluation of programmes of care.

Maintain accurate and up to date records, care plans and any appropriate documentation.

Report issues of clinical concern to the Band 6 Nurse or medical staff as appropriate.

Deal sympathetically with all complaints and report all complaints immediately to the Band 6 Nurse.

Promote positive healthcare and maintenance of good health through the education of clients and their families.

Take responsibility for the confidentiality of client related information.

Provide clinical advice, support, education and information to colleagues where clinical knowledge has developed through education and training.

Be aware of all issues relating to Risk Management and Health and Safety.

Through supervision from the Band 6 Nurse, ensure all clients on the caseload have a multi-disciplinary assessment of risk.

Create, maintain and enhance effective working relationships within a multi-disciplinary/multi-agency framework.

Work in collaboration with other agencies and services (including primary and secondary healthcare services), providing the appropriate clinical advice, support and training.

Be aware of their legal responsibilities with regard to client access to records and the Data Protection Act.
