The Caseload Holder will manage a defined caseload in line with good practice with responsibility over a 24 hour period.

The post holder will be involved in the complex needs assessment i.e. CHC, Health and Social Care and the provision of Specialist District Nursing Practice.

The post holder will be expected to work collaboratively with other agencies for the provision of patient services without direct supervision.

The post holder will be involved and expected to undertake a range of management duties and deputise in the absence of the Team Leader.

The post holder should hold a Specialist Practice Qualification in District Nursing , be working towards it and be a registered Nurse Prescriber or working towards, or willing to undertake the Specialist Practice Qualification in District Nursing next intake.

Main duties of the job

An opportunity has arisen for a Band 6, District Nurse Caseload Holder in the Dwyfor Area with the Eifionydd District Nursing Team.

Applications are invited from those who hold the District Nurse Specialist Practitioner Qualification, working towards the qualification or willing to commence the course at next intake. You will be responsible to the Team Leader and accountable for a designated caseload of patients.

Strong leadership and organisational skills are essential with the ability to work in busy, pressured environments whilst ensuring delivery of a high standard of nursing care.

You will be expected to assist in leading a pro-active, multi-skilled District Nursing Team committed to preventing hospital admissions and ensuring delivery of nursing excellence.

You should have evidence of continued professional development.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Qualifications & or Knowledge

NMC RN first level nurse
DN Specialist practice qualification / certificate, diploma, degree or working towards
Nurse prescribing qualification or working towards
Evidence of ongoing education and professional development
Mentorship and Assessment training

Desirable criteria

Management qualification eg ILM, RCN Leadership

Experience

Essential criteria

Community experience as band 5 or equivalent
Experience of working with patients who have acute / complex needs
Varied clinical experience and multi disciplinary working

Essential criteria

Analytical skills and ability to interpret clinical signs
Organisational skills
Abilty to assess, plan, implement, evaluate and delegate appropriately the delivery of care
Supervise teach and assess registered and non registered staff
Provide mentorship and prementorship as appropriate
Ability to work collaboratively with member of the primary health care team and other agencies
IT skills
Awareness of current research and its application to practice
Knowledge and evidence of application consent
Knowledge of data protection and pt confidentiality
Understanding of risk assessment and health & safety issues
Awareness of the changing health agenda which affects community practice

Desirable criteria

Experience of undertaking audits
Recognised qualification within an area of chronic conditions management
